Consent to medical/surgical procedures?

This may be state specific. While in some states, you can be as young as 12 in order to be the one to give permission for medical treatmt for such things as ually transmitted diseases, mental health treatment and drug treatment. To refuse medical treatment is another thing. The law is typically very narrow when it comes to a minor refusing treatment since its more likely that they do not have the complete mental maturity to make such decisions.
